Good day,
Well, here is the deal. I am 36 years old and my wife is 36 as well. I had a vasectomy and then a year later I had that reversed..( I might add that I was an idiot for doing that..) (and ouch I might add) :-) I have been to the doctor and had my sperm checked and I get the all normal reading and that I have about 85% mobility. and the color and the sperm is good, blah blah. I have done this test 3 times. Better safe to test more than once I think.. hehe. My wife has had her hormones checked. all good there. She has taken a form of clomid and nothing. We are in the process of thinking of IUI and the next steps. What am I expecting or is there any expectations with IUI? Does she need to have a controlled ovulation in the timeframe? I may just be talking out loud but I just want to know if this is a safe and effective way to go. If so, what are the normal rates for IUI? and what are the normal rates for IVF, if we go that way.
It is my passion for Heather and I to have another child, but only if it is God's will. I am in support of all her dreams and needs but as her husband and my friend I want this to be an effective and no harm to her method.

Thanks for the message. Let me try and address your questions, but feel free to make a face to face appointment.
1) I'm glad to hear that your sperm numbers are good. However, occasionally antibodies to sperm can form, so I would recommend you go to a fertility center and have a sperm antibody test. Also, fertility centers sometimes do a more careful evaluation of the various sperm parameters. Premier Fertility would be happy to help if you so desire. Either your local doctor can order the test through us or you can call us directly.
2) If both your testing and her testing are normal, then the category would be unexplained infertility. Of course, I can't say this until I have looked at all the testing. Unexplained infertility can be very frustrating since there is no clear reason for the infertility.
3) Treatment options for unexplained infertility are fertility drugs with insemination (30-40% pregnancy rate over 3-4 tries) versus IVF (success rate based on your wife's egg quality). If she has excellent egg quality, there is a roughly 50% success rate with 1 try.
4) The decision to proceed with inseminations versus IVF often depends on age, insurance, and the actual diagnosis. IVF works much better, but costs more. Obviously, if your sperm numbers are not normal or if she has pelvic or tubal disease, we don't waste time with inseminations but go straight to IVF.
5) Regarding safety, both options are very safe. The main issues to discuss are a possible multiple birth and the rare complication of an egg retrieval.
As you can probably tell from the above, this can be a complex discussion. You may want to make a face to face appointment. At the new patient appointment, you will meet with me for a consult (i.e. no procedures at the first visit) followed by a meeting with our financial counselor to review your insurance situation.
