Reply with quote | #1 | Dr. Deaton,
I've been off BCP since Oct 2008. In January of 2009, I began having MUCH lighter cycles (2 days and LIGHT the whole time) and in February they began to become erratic. My cycle in Feb was 2 days early. In March, it was late (due to stress, no doubt). In April, it was fairly on time; however, in May and June it was 2 weeks late. In July, it was a week late. I had my last period on August 1 (2 days early) and I have not had one since. I had an ultrasound on August 19 because I have a lump in my abdomen and my ob/gyn wanted to check it out. From this ultrasound, it was determined (by the tech) that my uterine lining was "super" thick and that I had a dominant follicle, but there was nothing blocking me internally from getting pregnant. My ob/gyn concluded that the lump in my stomach is *most likely* caused by having 2 abdominal surgeries within 8 weeks (a c-section with our daughter and then an appendectomy 8 weeks to the day later). When I asked him about the thickness of my uterine lining, he said that it was "objective" and then brushed me off. He offered Clomid to me in that same meeting, without doing any bloodwork, and I declined.
Since the ultrasound showed a dominant follicle, my husband and I went home and "got busy"...although I only had *some* eggwhite cervical fluid and very little ... desire ... (which my ob/gyn also said both were "objective"). I spotted on the Monday before my period was due (on Sunday) with a clot that night. I went back to spotting on Tuesday and by Wednesday morning, everything was completely gone. I spotted when pregnant with our daughter, so I was EXTREMELY hopeful...but got negative results from a HPT and a BPT taken that Thursday. The doctor gave me Provera, which I started that night and took through Labor Day Monday (he again, at this time, offered Clomid and I again declined). I began having PMS symptoms on Wednesday (increased hunger and migraine) and began spotting on Thursday, had a clot on Friday morning, but returned to spotting late Friday morning. I called the office and told them what was happening and the doctor said to give it another week. I told him I was ready for Clomid (after MUCH prayer) when I did start my period and the nurse told me this time that I need to wait at least another month but did not give me a reason (and it was the same nurse that I had been speaking with each time I called)...The spotting cleared up by Sunday and when I called on Monday, I was told that they did not have time to answer my questions...although not only is my doctor an ob/gyn, but an RE. Aside from the spotting, my breasts are tingly and seem heavier. I have lower abdominal pressure and *some* mild cramping. My husband and I have not had sex since that weekend so I know that I couldn't have gotten pregnant since then.
At this point, I just want to find out what is wrong with me so I can move forward and get pregnant. It did take us a year to get pregnant with our daughter, but I was NOT having these problems. I feel like my doctor is not listening or taking me seriously. I know my body pretty well, so I know when there is something wrong. What do I do at this point? Please help...

Thanks for the message. Sounds like you are getting frustrated, which is very understandable. Here are some thoughts:
1) Based on what you told me, it sounds like you are not ovulating. I would agree with an oral fertility drug, either clomid or femara.
2) If there is a thick endometrial lining on ultrasound, then it is helpful to shed it before you start fertility drugs. Provera usually works, but if not then prometrium can be tried. If the ultrasound shows a thin lining, you can start the clomid or femara without having a period.
3) I would also recommend some routine bloodwork, such as prolactin and TSH, before you start. We often do a test for egg quality, such as AMH.
4) If you have a child and nothing else is wrong except for the ovulation disorder, you should have an excellent chance on fertility drugs. If the oral meds don't work, then injectible fertility drugs would be the next step.
Hope this helps. Blessings. __________________ Jeffrey L. Deaton, M.D.
