Intralipid Therapy

Dr. Deaton- I have had 7 consecutive m/c. Three of my pregnancies were karyotyped as normal. I have had the full RPL workup and tested normal for everything. Recently I had an immune workup done for RPL, which showed I have elevated NK cell activity. The clinic I have been with for the last two years does not use intralipid therapy or IVIG. My last pregnancy I was treated empirically with lovenox and prometrium. Heartbeat was seen at 6w4d, 7w4d, 8w4d and growth stopped at 9w2d. Have you prescribed intralipid therapy to any of your patients?
Thanks,
Amy-Lynn
need some advise

Good day,

Well, here is the deal. I am 36 years old and my wife is 36 as well. I had a vasectomy and then a year later I had that reversed..( I might add that I was an idiot for doing that..) (and ouch I might add) :-) I have been to the doctor and had my sperm checked and I get the all normal reading and that I have about 85% mobility. and the color and the sperm is good, blah blah. I have done this test 3 times. Better safe to test more than once I think.. hehe.
My wife has had her hormones checked. all good there. She has taken a form of clomid and nothing.
We are in the process of thinking of IUI and the next steps. What am I expecting or is there any expectations with IUI? Does she need to have a controlled ovulation in the timeframe? I may just be talking out loud but I just want to know if this is a safe and effective way to go. If so, what are the normal rates for IUI? and what are the normal rates for IVF, if we go that way.

It is my passion for Heather and I to have another child, but only if it is God's will. I am in support of all her dreams and needs but as her husband and my friend I want this to be an effective and no harm to her method.

Thanks for letting me ramble. Any/all info is appreciated.

Thanks and have a blessed day.

Kerry and Heather McArthur

need advice

Hi Dr. Deaton,

I have been taking Depo Provera shots for the last 5 years for birth control. My husband and I were married in April 2009. We both decided we didn't want to "wait" to start our family. So I took the last birth control shot in April. The next shot would have been due at the end of June, but I canceled my appointment and didn't get the Depo shot. In August, I started my first period in close to four years. I was excited because I had heard so many horror stories of people coming off Depo and not getting their period for a year or more after their last shot. September came around and I had a light period, but had it twice that month. In October my period came and lasted seven days. Eight days later it came back again. I am concerned that I am not ovulating because of how sporadic my cycle has been.
No ovulation = no baby

Also, in the months since coming off of Depo I have been an emotional rollercoaster. I am accounting this to the hormones in my body. I am up, then down, and the littlest thing sets me off. This is abnormal. I am a happy-go-lucky type person, with a bubbling personality and for the past few months that has been occasionally masked with something else.

So, I have the irregular and frequent period and the emotional rollercoaster issues going on. What is your advice to regulate my cycle and take a hold of these crazy hormones so that my husband and I have a shot of getting pregnant?
Also, what have you seen with patients who stop taking Depo? Do they have a more difficult time conceiving?

Thank you for any and all advice.

Low Progestrone

Hi Dr. Deaton,
I had bloodtest taken and showed that I had low progestrone. They put me on Clomid and my body reacted very well and my progestrone normalized. However, I did not get pregnant on it during the 6 months. Do you think that I would be a good candidate for IUI? My husbands sperm count is normal. Thanks for your help.

mjeune
What to do?

Dr. Deaton,

I've been off BCP since Oct 2008. In January of 2009, I began having MUCH lighter cycles (2 days and LIGHT the whole time) and in February they began to become erratic. My cycle in Feb was 2 days early. In March, it was late (due to stress, no doubt). In April, it was fairly on time; however, in May and June it was 2 weeks late. In July, it was a week late. I had my last period on August 1 (2 days early) and I have not had one since. I had an ultrasound on August 19 because I have a lump in my abdomen and my ob/gyn wanted to check it out. From this ultrasound, it was determined (by the tech) that my uterine lining was "super" thick and that I had a dominant follicle, but there was nothing blocking me internally from getting pregnant. My ob/gyn concluded that the lump in my stomach is *most likely* caused by having 2 abdominal surgeries within 8 weeks (a c-section with our daughter and then an appendectomy 8 weeks to the day later). When I asked him about the thickness of my uterine lining, he said that it was "objective" and then brushed me off. He offered Clomid to me in that same meeting, without doing any bloodwork, and I declined.

Since the ultrasound showed a dominant follicle, my husband and I went home and "got busy"...although I only had *some* eggwhite cervical fluid and very little ... desire ... (which my ob/gyn also said both were "objective"). I spotted on the Monday before my period was due (on Sunday) with a clot that night. I went back to spotting on Tuesday and by Wednesday morning, everything was completely gone. I spotted when pregnant with our daughter, so I was EXTREMELY hopeful...but got negative results from a HPT and a BPT taken that Thursday. The doctor gave me Provera, which I started that night and took through Labor Day Monday (he again, at this time, offered Clomid and I again declined). I began having PMS symptoms on Wednesday (increased hunger and migraine) and began spotting on Thursday, had a clot on Friday morning, but returned to spotting late Friday morning. I called the office and told them what was happening and the doctor said to give it another week. I told him I was ready for Clomid (after MUCH prayer) when I did start my period and the nurse told me this time that I need to wait at least another month but did not give me a reason (and it was the same nurse that I had been speaking with each time I called)...The spotting cleared up by Sunday and when I called on Monday, I was told that they did not have time to answer my questions...although not only is my doctor an ob/gyn, but an RE. Aside from the spotting, my breasts are tingly and seem heavier. I have lower abdominal pressure and *some* mild cramping. My husband and I have not had sex since that weekend so I know that I couldn't have gotten pregnant since then.

At this point, I just want to find out what is wrong with me so I can move forward and get pregnant. It did take us a year to get pregnant with our daughter, but I was NOT having these problems. I feel like my doctor is not listening or taking me seriously. I know my body pretty well, so I know when there is something wrong. What do I do at this point? Please help...

Sorry for the LONG post!!!!!
Rhonda
Ectopic Pregnancy

My husband and I have been trying to conceive for over 6 years. Last year, I finally got pregnant on my own, no medication or anything. It turned out to be a misscarriage. I finally got my OBGYN to refer me to a specialist after another year went by and did not get pregnant again. My bloodwork was fine, my cycles are between 24-28 days regularly, and I ovulate each month. My husband did a semen analysis and his was normal (only thing was he had a higher % of abnormal shapped), but doc said it wasn't that abnormal where he would be worried about it. I did an HSG and my one tube was totally blocked and the other showed the dye go in but not spill out. So the doc thinks that my last misscarriage was ectopic due to my symptoms and my tubes. After our last follow up to go over the results he suggested that I do a laparscopy and he said his best diagnosis is that I have Endometriosis. After reading on the internet... it seems as if my best option would be to skip the laparscopy and go straight to IVF. Am I right in thinking this? I'm not trying to rush over anything, however found that if I was ectopic my chances are higher to have another one... also the expense of it and chances of it working are so low? Thanks for your help.

Hi Dr.Deaton.
I'm in the marketing for a good infertility doctor. Though, I don't have a problem getting pregnant, my last 3 losses stopped @ 7wks. I have been to Lynnhurst and Hawthorne and both places, I've loved but have ended up with bad experiences... Dr is either cold, monotoned and treating me as a number.

I would like to know after all the basic tests are done, what other tests could you consider to be performed?? I've had hormonal abnormailities, metabolic, uterine, antiphospholipid(positive), thrombophilias tests done. I did take heparin and aspirin on my last pregnancy, but that did not solve the recurrent problem. Currently, awaiting chromsomal analysis results.

Also, I'm always thinking about the problem I am having so could the IUD for 5 years, thining of the uterus lining cause my miscarriages?

3362937429(HM) or I can await your response here.
I am 40 years old, recently remarried and we have been trying to conceive. I have had two successful pregnancies (age 20, 19) and in April 09 had a blighted ovum. My husband is on Testosterone replacement therapy, which we have been told could reduce the sperm count. We have been trying since the miscarrage in April without success. We are looking for any information reguarding the next steps to follow considering the advanced age, miscarrage and the testosterone replacement. Any information you can provide will be most appreciated.

Thank you
Dr. Deaton,
My husband and I were in to see you for our initial consultation in early July due to my elevated fsh levels (measured 18). You instructed me to return to the clinic on day 3 of my menstrual cycle for an fsh level and amh level so that we could see what our options for treatment may be. I have not been able to be tested due to the fact that I have not had a period since June 27. I have taken multiplte home pregnancy tests all of which have been negative. I haven't had any symptoms of pregnancy other than right low back pain, which actually has me a little concerned with my previous history an ectopic. Should I be concerned? I haven't ever completely missed a cycle.
Thank You
